Liability Waiver and Participation Agreement

 

Waiver and Release of Liability

 

By signing this waiver, I acknowledge and agree to the following terms and conditions for myself and the child(ren) listed above while participating in open play at Grit Studio & Collaborative – Mindful Mamas Open Play:

1. Assumption of Risk

I understand that participation in play activities involves inherent risks, including but not limited to injury or illness. I voluntarily assume full responsibility for any risk of harm or loss associated with these activities.

2. Supervision of Children

I understand and agree that Grit Studio & Collaborative does not provide supervision for children. I am solely responsible for supervising my child(ren) at all times. I agree to remain on the premises and attentive to my child(ren)’s needs and behavior during our visit.

3. Clean-Up and Personal Responsibility

I agree to clean up after myself and my child(ren), including any food, toys, or general mess. I acknowledge that I am responsible for any damage or disruption caused by myself or my child(ren).

4. Space Boundaries

I understand that photo or video sessions may occur during open play hours. Children must remain in the designated front play area unless accompanied to the restroom. Entry into other studio areas is not permitted unless authorized.

5. Release of Liability

I hereby release and discharge Grit Studio & Collaborative, its owners, staff, volunteers, and affiliates from any and all liability, claims, demands, or causes of action that may arise from our participation.

6. Indemnification

I agree to indemnify and hold harmless Grit Studio & Collaborative from any claims or liabilities that arise from our actions, including injury, illness, or property damage caused by myself or my child(ren).
